Step 1: Download, fill out, and sign the parent contract. Only one contract per family is needed.
Step 2: Download, fill out, and sign the permission waivers. One of these is required for each student.
Step 3: Download, fill out, and sign the medication authorization. These are only necessary if you have a student who will need to take medication while at camp.
Step 4: Click the link below to register your student. Be sure to fill all the info out for all of your students going!
Step 5: Choose how you want to pay.
Pay by check: please fill out the provided Payment Worksheet below and bring it to the church along with a check written out to “West Newbury Congregational Church.”
Pay by card: please fill out the provided Payment Worksheet, email it to koinoniayascamp@gmail.com, and click the button to pay online.
Be sure to select Koinonia YAS Camp as the correct recipient. For the convenience of paying online, we also ask that you select “Cover Fees”
